We just had another experience with the same site, SM City Seaside, when we went to get our adopted daughter's first passport.  We brought every document listed on the website but they asked us for our daughter's original birth certificate, pre-adoption version.  There was no negotiating with them on this as they refused to process the application without it.

There is a mention of PSA docs required for a name change so perhaps that is where this comes from?

There was no name change involved as this was the first time ever applying for a passport for her.  The birth certificate provided to us by PSA has us as the parents and her with our last name.

There is a question or there use to be, on the passport renewal that asked if you have a valid foreign passport and what it's number is. Seems reasonable they might want to check the foreign passport to make sure the information entered on the application was correct.

That isn't on there anymore.

Currently the DFA doesn't ask for a foreign passport number but does ask about other citizenship:

I did encounter an unexpected request from DFA while renewing passports for dual citizenship children.  The DFA wanted to see the foreign parent's passport.  Wasn’t on any requirement list, and easy to comply. Just not expected.

Thanks for the tip.  Its good to know a little grease will  still get the wheels rolling.   I need to get my new son a PH Passport ... as I dont want to pay the overstay penalties  when we travel in May.
