If You Have School Age Children

Just registered our daughter for school here in Florida. Had to make sure all of her shots were up to date. Turns out her shot record book is filled with shots given on 3/12, 4/16, 8/23. needless to say the health department wouldn't accept them.  3/17 is not a date, it is just meaningless numbers. 3/17/2014 is a date. you would think a doctor or a nurse would know this. My daughter had to get shots that she didn't need because of some idiot not knowing how to write a date. Not only in the Philippines.

This is good information for anyone bringing a child to US. We also registered our child in Fl but (thankfully) all shot dates had the specific date so were accepted.

I doubt the specific date of shot would be important. But the floridahealth.gov site states that all vaccine administration dates on the DH 680 (shot record) must be in month/day/year format.
